In Reply We thank Nagel et al for their thoughtful comments on our article on medical management vs mechanical thrombectomy (MT) for mild strokes.1 We agree that medically treated patients with large vessel occlusion (LVO) with mild deficits can potentially experience neurological deterioration and rescue therapy may be required. We do not know how often this occurs or if there are factors that might increase or decrease this rate.
