In 2020, the strongest predictor of the outcome for a child with cancer is the country in which he or she is born. Massive inequalities in paediatric cancer care are a blemish on a global cancer enterprise that too often celebrates marginal wins. The Lancet Oncology Commission on sustainable care for childhood cancer is therefore a welcome call to arms for the oncology community. The Commission presents new evidence to justify investment in and a framework to guide the scale-up of interventions for childhood cancers in low-income and middle-income countries (LMICs). This work complements growing global interest in the Cancer Groundshot and Choosing Wisely initiatives, which seek to optimise population health by implementing elements of care that are already known to save lives and avoiding care that is not helpful and might be harmful.
