This issue of the journal contains a study by Garza et al1 that reports a symptom-modifying effect of injection of adipose-derived stromal vascular fraction cells in patients with knee osteoarthritis. A strength of the study is the multisite, prospective, randomized, double-blind, placebo-controlled trial design. The authors demonstrate significant improvement in symptoms using standardized outcomes scales. However, MRI scans demonstrated no significant structural changes in the treated joints. The study demonstrates that cell therapy formulations may be symptom-modifying, but true tissue regeneration is elusive. It is well-established that mesenchymal stem cells and other progenitor cell populations can produce various immunomodulatory, anti-inflammatory, and other trophic factors that can have a positive effect on symptoms via paracrine mechanisms. However, patients are attracted to the concept of “stem cells” and “regenerative medicine” by the lure of tissue regeneration and rejuvenation, despite minimal scientific evidence that injected or surgically implanted cells differentiate into relevant cell phenotypes or regenerate functional tissue.
