SEE PAGE 53 A mong the different manifestations of peripheral arterial disease (PAD), atherosclerosis of lower extremity arteries is a major health threat that is undertreated, and poorly understood by the medical community (1). Population studies have estimated the prevalence of PAD around 4% in both Western Europe and U.S. populations (2,3). Individuals with symptomatic PAD display a high risk of cardiovascular morbidity and mortality. PAD is a complex multifactorial occlusive disorder and currently there is a paucity of mechanisms available to explain the underlying pathogenesis of the disease. Therefore, a limited innovative framework for the diagnosis and treatment of patients has been established (4). Pioneer studies highlight the contribution of circulating platelets as unexpected cellular entities involved in the pathogenesis of symptomatic atherosclerosis in coronary artery disease, cardiovascular disease, and PAD (5–7). The role of platelets in atherosclerosis-related disorders was restricted initially to thrombus formation upon rupture of advanced atherosclerotic plaques.
