    We thank George Henderson and colleagues for their interest in our analysis. We agree that non-dietary factors such as genetic polymorphisms1 affect biomarker levels of the omega-6 polyunsaturated fatty acids (PUFAs) linoleic acid and arachidonic acid. However, increased dietary linoleic acid increases linoleic acid biomarker levels in trials,2 providing clear evidence that biomarker levels reasonably reflect dietary intake. We agree that compared with linoleic acid, arachidonic acid metabolism has a more prominent function than direct dietary intake in the regulation of biomarker levels.

    The findings of Jason H Y Wu and colleagues' pooled analysis1 of linoleic acid and arachidonic acid biomarkers and incident type 2 diabetes should be interpreted with caution. Linoleic acid and arachidonic acid in serum are derived from the diet, but levels are controlled by the rate of peroxidation to bioactive metabolites. Serum linoleic acid or arachidonic acid status can be strongly affected by alcohol intake or smoking, which promote the formation of such metabolites.2 The inflammatory potential of the overall diet can also affect fatty acid biomarkers.

    Luca Mascitelli and Mark R Goldstein speculate that the reduction in myocardial infarction reported in the FOURIER trial1 could have been driven by myocardial infarctions occurring in the setting of elective percutaneous coronary intervention. However, such an assumption is unfounded, since the medical literature clearly shows that universal myocardial infarction definition type 4a events (ie, myocardial infarction associated with revascularisation) only account for 1–2% of all myocardial infarctions in clinical trials of patients with stable vascular disease.

    The primary results of the FOURIER trial1 showed that the significant reduction in cardiovascular events in the evolocumab group was driven by revascularisation—an endpoint that is to some extent discretionary2—and non-fatal myocardial infarction, without indicating how many of these infarctions were periprocedural events that might have a different prognostic significance to spontaneous myocardial infarctions. Furthermore, loss of masking to treatment allocation could have occurred because evolocumab predictably lowers LDL cholesterol, and decisions about revascularisations might have been biased by physicians who knew patients' lipid measurements.

Urinary iodine concentration is a reliable biomarker of iodine status at the population level, but not at the individual level.1,2 Thus, whether all the pregnant women included in our study3 were iodine deficient remains unclear and exposure misclassification might have biased our results.

    HIV-positive populations in sub-Saharan Africa are both growing and ageing.1 These population changes are due in large part to the massive scale-up of antiretroviral therapy and its success in averting HIV-related mortality. One result of this scale-up has been a surge in the number of people living with HIV in the region.2 Mass HIV treatment has therefore contributed to a striking change in population structure; for example, average adult life expectancy in rural KwaZulu-Natal, South Africa, increased by 11·3 years between antiretroviral therapy roll-out in 2003 and 2011.

    In The Lancet Diabetes & Endocrinology, two new reports are of particular relevance to the clinical management of primary aldosteronism.1,2 The first, from Silvia Monticone and colleagues, is a systematic review and meta-analysis comparing patients with primary aldosteronism with those with essential hypertension at the time of presentation.1 Although the 31 observational studies are classified as prospective or retrospective, all look backward to differences between 3838 patients with primary aldosteronism versus 9284 with essential hypertension.

    Paricalcitol is a synthetic vitamin D receptor agonist licensed to treat secondary hyperparathyroidism, which was found to be nephroprotective in preclinical studies.1 In the VITAL clinical trial,2 the investigators reported that addition of 2 μg per day paricalcitol to renin–angiotensin system blockade safely lowered residual albuminuria in patients with diabetic nephropathy. However, in a post-hoc analysis, benefit was limited to patients with a high salt intake.

    Cushing's disease, pituitary-dependent Cushing's syndrome, is a rare disorder with an incidence in the region of 1–2 cases per million population per year worldwide1—but is of interest far beyond its rarity: it causes specific problems in diagnosis and can be difficult to treat effectively, but successful therapy can show spectacular clinical transformations. It is most often caused by a small corticotroph tumour, often just a few mm in diameter. In classic cases it presents with florid unmistakable manifestations, although much more subtle cases are increasingly being recognised.

    The outdated designations of type 1 diabetes as “juvenile-onset diabetes” and type 2 diabetes as “adult-onset diabetes” reflect the incorrect categorical paradigm that has hindered the ability of clinicians to correctly diagnose outlier cases, causing unnecessary delays in instituting the appropriate therapy. The age-based distinction between the two most common forms of diabetes has become increasingly blurred, as type 2 diabetes becomes more frequent at younger ages and autoimmune diabetes in adulthood is better recognised.

    Cancer is the second leading cause of death from non-communicable diseases in the world.1 In 2015, there were 17·5 million new cases of cancer and 8·7 million cancer-related deaths, with 208·3 million disability-adjusted life-years.2 Cancer incidence is expected to continue to increase, partly due to changes in the prevalence of risk factors such as obesity, diabetes, and other lifestyle factors. Obesity, defined as body-mass index (BMI) of 30 kg/m2 or more, has become increasingly common in all age groups and every country over the past few decades, irrespective of the country's sociodemographic status or region.

  • Effect of vitamin D supplementation on non-skeletal disorders: a systematic review of meta-analyses and randomised trials

    Randomised trials reported up to Dec 31, 2012, did not confirm that vitamin D supplementation could protect from non-skeletal health conditions affecting adults, as was expected on the basis of data from observational studies. To examine whether the more recently published meta-analyses and trials would change past conclusions, we systematically reviewed meta-analyses of vitamin D supplementation and non-skeletal disorders published between Jan 1, 2013, and May 31, 2017, that included study participants of all ages, including pregnant women. We also searched for randomised trials not included in meta-analyses. We identified 87 meta-analyses, of which 52 were excluded because they contained less recent literature or were of suboptimal quality. We retrieved 202 articles on trials that were not included in meta-analyses. Recent meta-analyses reinforce the finding that 10–20 μg per day of vitamin D can reduce all-cause mortality and cancer mortality in middle-aged and older people. Although vitamin D doses were greater than those assessed in the past, we found no new evidence that supplementation could have an effect on most non-skeletal conditions, including cardiovascular disease, adiposity, glucose metabolism, mood disorders, muscular function, tuberculosis, and colorectal adenomas, or on maternal and perinatal conditions. New data on cancer outcomes were scarce. The compilation of results from 83 trials showed that vitamin D supplementation had no significant effect on biomarkers of systemic inflammation. The main new finding highlighted by this systematic review is that vitamin D supplementation might help to prevent common upper respiratory tract infections and asthma exacerbations. There remains little evidence to suggest that vitamin D supplementation has an effect on most conditions, including chronic inflammation, despite use of increased doses of vitamin D, strengthening the hypothesis that low vitamin D status is a consequence of ill health, rather than its cause. We further hypothesise that vitamin D supplementation could exert immunomodulatory effects that strengthen resistance to acute infections, which would reduce the risk of death in debilitated individuals. We identified many meta-analyses of suboptimal quality, which is of concern. Future systematic reviews on vitamin D should be based on data sharing so that data for participants with the same outcomes measured in the same way can be pooled to generate stronger evidence.

    We agree that it could be of interest to investigate the relation between hyperthyroidism and atrial fibrillation in patients with type 1 diabetes. However, we do not have the International Classification of Diseases codes available in the database used for our study. In our Article,1 we attempted to emphasise that atrial fibrillation is more common in patients with type 1 diabetes than in controls, but that the cause of this association needs to be investigated further. Notably, although the relative excess risk of atrial fibrillation compared with controls was greater for women than for men with diabetes, the absolute risk was somewhat greater for men than for women.

    We read the Article by Sofia Dahlqvist and colleagues with great interest.1 Their study reports, for the first time, an increased risk of atrial fibrillation in patients with type 1 diabetes, and showed that the increased risk was associated with renal complications and poor glycaemic control. These findings will be of substantial interest to clinicians.

    The most common complication in people who are born small for gestational age is persistent short stature: approximately 10% of children born small for gestational age will remain less than 2 SDs for height throughout childhood and adulthood. As a result, such children who do not have suitable catch-up growth during the first years of life and who are growing at an average or below average rate are candidates for treatment with recombinant growth hormone.1 Being born small for gestational age is also associated with an increased risk of cardiometabolic disorders in adulthood, including diabetes, dyslipidaemia, and cardiovascular disease.

    In The Lancet Diabetes & Endocrinology, Kohjiro Ueki and colleagues1 report the results of J-DOIT3, a multicentre, randomised clinical trial in which 2542 Japanese patients with type 2 diabetes aged 45–69 years were randomly assigned to intensive treatment (n=1271) to achieve new targets for blood pressure (<120/75 mm Hg), LDL cholesterol (<80 mg/dL or <70 mg/dL, depending on history of cardiovascular disease), and HbA1c (6·2% [44·3 mmol/mol]), or targets recommended by the Japanese Diabetes Society (blood pressure <130/80 mm Hg, LDL cholesterol <120 mg/dL or <100 mg/dL, and HbA1c <6·9% [52·0 mmol/mol]; n=1271) for a median of 8·5 years (IQR 7·3–9·0).

    The focus of cardiovascular disease prevention has shifted from normalisation of risk factors to absolute risk reduction. Reducing LDL cholesterol concentration by 1 mmol/L for about 5 years is consistently associated with a 23–25% lowered risk of major cardiovascular events, with statin and non-statin therapies alike, irrespective of baseline LDL cholesterol concentration.1 In high-income and middle-income countries, statins are now routinely recommended as first-line LDL cholesterol-lowering therapy for people with 10-year modelled risk above, somewhat arbitrary, country-specific thresholds defined by economic and clinical considerations.

    Non-communicable diseases (NCDs) are an increasing cause of morbidity and premature mortality worldwide, requiring countries to adapt health systems and policies towards population health strategies.1 NCDs at the population level can have acute alterations in prevalence and outcomes over time. Abrupt changes in social and economic conditions can bring on rapid shifts in risky behaviours, such as alcohol misuse or tobacco use, that are precursors to many NCDs. Current periodic survey-based methods to monitor NCDs and risk factors might not be adequate to identify these acute changes.
