First page of articleOver two decades, there has been debate about whether there is a specific cardiovascular risk associated with high increments in postload glucose concentrations, over and above the risk conferred by dysglycaemia (slightly raised plasma glucose concentrations not necessarily diagnostic for diabetes) or hyperglycaemia (abnormally high plasma glucose concentrations compatible with a diagnosis of diabetes). Are the results of oral glucose tolerance testing or glycaemic excursions after mixed meals1 more important than fasting glucose and HbA1c concentrations? Epidemiological data suggest that postload glycaemic excursions are more closely linked to cardiovascular events,1 but even slightly raised fasting plasma glucose concentrations are predictive of cardiovascular events as well.
