    Non-communicable diseases (NCDs) are an increasing cause of morbidity and premature mortality worldwide, requiring countries to adapt health systems and policies towards population health strategies.1 NCDs at the population level can have acute alterations in prevalence and outcomes over time. Abrupt changes in social and economic conditions can bring on rapid shifts in risky behaviours, such as alcohol misuse or tobacco use, that are precursors to many NCDs. Current periodic survey-based methods to monitor NCDs and risk factors might not be adequate to identify these acute changes.

    With a plethora of new drugs for diabetes, and distinct differences in their mechanisms of action and non-glycaemic effects, the selection of a second-line medication to add to metformin is now a complex decision. Guidelines1,2 offer some suggestions for individualisation of treatment, based on efficacy, side-effects, and cost. But the appropriate choice might be challenging for clinicians, patients, and payers, and older (and cheaper) drugs such as sulfonylureas and thiazolidinediones remain in use despite concerns about side-effects.

    Life expectancy is significantly reduced in people with type 1 diabetes, and quality of life is frequently affected by eye, kidney, foot, and cardiovascular disease. Achieving and sustaining control of blood glucose to reduce the risk of these complications is a substantial challenge for many affected individuals. One strategy under investigation is the repurposing of drugs used in other conditions as adjunct therapy.1 An ideal adjunct agent—added to insulin therapy—would provide a sustained reduction in blood glucose concentrations without increasing hypoglycaemia, while preventing insulin-induced weight gain and improving cardiovascular outcomes.

    Paricalcitol is a synthetic vitamin D receptor agonist licensed to treat secondary hyperparathyroidism, which was found to be nephroprotective in preclinical studies.1 In the VITAL clinical trial,2 the investigators reported that addition of 2 μg per day paricalcitol to renin–angiotensin system blockade safely lowered residual albuminuria in patients with diabetic nephropathy. However, in a post-hoc analysis, benefit was limited to patients with a high salt intake.

    The most common complication in people who are born small for gestational age is persistent short stature: approximately 10% of children born small for gestational age will remain less than 2 SDs for height throughout childhood and adulthood. As a result, such children who do not have suitable catch-up growth during the first years of life and who are growing at an average or below average rate are candidates for treatment with recombinant growth hormone.1 Being born small for gestational age is also associated with an increased risk of cardiometabolic disorders in adulthood, including diabetes, dyslipidaemia, and cardiovascular disease.

A patient-centred approach is a cornerstone of high-quality diabetes care and is mirrored in research by an increasing focus on patient and public involvement.2 The James Lind Alliance has a 13-year history of collaboration with patients, carers, and clinicians in identifying and prioritising unmet research needs to drive improvements in clinical care.

  • Effect of vitamin D supplementation on non-skeletal disorders: a systematic review of meta-analyses and randomised trials

    Randomised trials reported up to Dec 31, 2012, did not confirm that vitamin D supplementation could protect from non-skeletal health conditions affecting adults, as was expected on the basis of data from observational studies. To examine whether the more recently published meta-analyses and trials would change past conclusions, we systematically reviewed meta-analyses of vitamin D supplementation and non-skeletal disorders published between Jan 1, 2013, and May 31, 2017, that included study participants of all ages, including pregnant women. We also searched for randomised trials not included in meta-analyses. We identified 87 meta-analyses, of which 52 were excluded because they contained less recent literature or were of suboptimal quality. We retrieved 202 articles on trials that were not included in meta-analyses. Recent meta-analyses reinforce the finding that 10–20 μg per day of vitamin D can reduce all-cause mortality and cancer mortality in middle-aged and older people. Although vitamin D doses were greater than those assessed in the past, we found no new evidence that supplementation could have an effect on most non-skeletal conditions, including cardiovascular disease, adiposity, glucose metabolism, mood disorders, muscular function, tuberculosis, and colorectal adenomas, or on maternal and perinatal conditions. New data on cancer outcomes were scarce. The compilation of results from 83 trials showed that vitamin D supplementation had no significant effect on biomarkers of systemic inflammation. The main new finding highlighted by this systematic review is that vitamin D supplementation might help to prevent common upper respiratory tract infections and asthma exacerbations. There remains little evidence to suggest that vitamin D supplementation has an effect on most conditions, including chronic inflammation, despite use of increased doses of vitamin D, strengthening the hypothesis that low vitamin D status is a consequence of ill health, rather than its cause. We further hypothesise that vitamin D supplementation could exert immunomodulatory effects that strengthen resistance to acute infections, which would reduce the risk of death in debilitated individuals. We identified many meta-analyses of suboptimal quality, which is of concern. Future systematic reviews on vitamin D should be based on data sharing so that data for participants with the same outcomes measured in the same way can be pooled to generate stronger evidence.

    Follicular thyroid cancer is the second most common differentiated thyroid cancer histological type and has been overshadowed by its more common counterpart—papillary thyroid cancer—despite its unique biological behaviour and less favourable outcomes. In this Review, we comprehensively review the literature on follicular thyroid cancer to provide an evidence-based guide to the management of these tumours, to highlight the lack of evidence behind guideline recommendations, and to identify changes and challenges over the past decades in diagnosis, prognosis, and treatment. We highlight that correct identification of cancer in indeterminate cytological samples is challenging and ultrasonographic features can be misleading. Despite certain unique aspects of follicular thyroid cancer presentation and prognosis, no specific recommendations exist for follicular thyroid cancer and Hürthle cell carcinoma in evidence-based guidelines. Efforts should be made to stimulate additional research in this field.

    In The Lancet Diabetes & Endocrinology, Kohjiro Ueki and colleagues1 report the results of J-DOIT3, a multicentre, randomised clinical trial in which 2542 Japanese patients with type 2 diabetes aged 45–69 years were randomly assigned to intensive treatment (n=1271) to achieve new targets for blood pressure (<120/75 mm Hg), LDL cholesterol (<80 mg/dL or <70 mg/dL, depending on history of cardiovascular disease), and HbA1c (6·2% [44·3 mmol/mol]), or targets recommended by the Japanese Diabetes Society (blood pressure <130/80 mm Hg, LDL cholesterol <120 mg/dL or <100 mg/dL, and HbA1c <6·9% [52·0 mmol/mol]; n=1271) for a median of 8·5 years (IQR 7·3–9·0).

    Cushing's disease, pituitary-dependent Cushing's syndrome, is a rare disorder with an incidence in the region of 1–2 cases per million population per year worldwide1—but is of interest far beyond its rarity: it causes specific problems in diagnosis and can be difficult to treat effectively, but successful therapy can show spectacular clinical transformations. It is most often caused by a small corticotroph tumour, often just a few mm in diameter. In classic cases it presents with florid unmistakable manifestations, although much more subtle cases are increasingly being recognised.

    Diabetes is an important cardiovascular risk factor, but until recently a disconnect has existed between management of type 2 diabetes and reduction of cardiovascular mortality and morbidity. Such was the mixed impact of previous generations of antihyperglycaemic drugs that the US and European drug regulators mandated cardiovascular outcome studies to assess the safety of all future therapeutic interventions. Somewhat ironically, this move unveiled an unexpected but welcome cardiovascular benefit with some newer generation drugs, including the sodium-glucose co-transporter-2 (SGLT2) inhibitors.

  • Diabetes structured self-management education programmes: a narrative review and current innovations

    Both type 1 and type 2 diabetes are associated with long-term complications that can be prevented or delayed by intensive glycaemic management. People who are empowered and skilled to self-manage their diabetes have improved health outcomes. Over the past 20 years, diabetes self-management education programmes have been shown to be efficacious and cost-effective in promotion and facilitation of self-management, with improvements in patients' knowledge, skills, and motivation leading to improved biomedical, behavioural, and psychosocial outcomes. Diabetes self-management education programmes, developed robustly with an evidence-based structured curriculum, vary in their method of delivery, content, and use of technology, person-centred philosophy, and specific aims. They are delivered by trained educators, and monitored for quality by independent assessors and routine audit. Self-management education should be tailored to specific populations, taking into consideration the type of diabetes, and ethnic, social, cognitive, literacy, and cultural factors. Ways to improve access to and uptake of diabetes self-management programmes are needed globally.

    We appreciate the interest in our Personal View and thank Katrine Hygum and colleagues for their comments. We also recognise the complexity and multifactorial nature of diabetic bone disease, but still find strong indications for the key role of microvascular disease in diabetic bone disease. Although the results of the Danish register-based study1 with data from more than 14 000 patients with diabetes suggested that neither diabetic eye disease nor neuropathy added to the risk of fractures, presence of diabetic kidney disease doubled the risk of fracture in comparison with patients with diabetes who had no microvascular disease.

    We welcome the timely Personal View on bone disease in diabetes by Vikram Shanbhogue and colleagues.1 We believe that the increased fracture risk in patients with diabetes is caused by multiple factors including insulin deficiency, hyperinsulinaemia, an altered incretin response, obesity, use of glucose-lowering drugs, use of antihypertensive and lipid-lowering drugs, comorbidities, falls, and diabetes complications. We suggest that interplay of these factors causes the increased fracture risk in patients with diabetes and that diabetic bone disease is not predominantly attributable to the presence of microvascular disease.

    We note Yu Zhang and Rongrui Tang's criticism of the use of a placebo control group in the original FREEDOM trial,1 which was followed by a 7-year open-label extension, the subject of our Article. FREEDOM was initiated in 2004, was conducted in compliance with Food and Drug Administration and European Medicines Agency regulatory guidance,2,3 and was reviewed and approved by the Ethics Committee or equivalent for every participating site. In addition, all participants were given calcium and vitamin D supplementation, and the protocol provided for discontinuation of study medication and initiation of appropriate treatment for participants who experienced an osteoporosis-related fracture or a significant decrease in bone mineral density.

    In The Lancet Diabetes & Endocrinology, Henry Bone and colleagues reported results of denosumab administration for up to 7 or 10 years in postmenopausal women with osteoporosis.1 Participants who had completed the randomised FREEDOM trial were offered to participate in the open-label, 7-year extension, receiving 60 mg of denosumab subcutaneously every 6 months.

    The focus of cardiovascular disease prevention has shifted from normalisation of risk factors to absolute risk reduction. Reducing LDL cholesterol concentration by 1 mmol/L for about 5 years is consistently associated with a 23–25% lowered risk of major cardiovascular events, with statin and non-statin therapies alike, irrespective of baseline LDL cholesterol concentration.1 In high-income and middle-income countries, statins are now routinely recommended as first-line LDL cholesterol-lowering therapy for people with 10-year modelled risk above, somewhat arbitrary, country-specific thresholds defined by economic and clinical considerations.
